A licensed Home Modification Assessor (HMA) will meet with you at your residence to discuss the unique challenges you face and offer solutions to meet your individualized needs. We have experience working with CADI, CAC, DD & BI waivers. Our HMA's also have construction backgrounds to ensure the recommendations they discuss, and present are realistic and attainable.
An assessment summary report along with any applicable designs will be presented for your review. The Home Modification Assessor (HMA) will submit your report along with a detailed Scope of Work for bidding purposes to your case manager for County review. Based on the County's feedback the Scope of Work may need to be edited to align with their request. This approach allows for knowing what recommended modifications are approved to be paid for by the waiver, allowing the contractors to bid only on items that can be covered.
Your Home Modification Assessor (HMA) will assist in the selection of qualified, enrolled contractors best suited for your project and will meet with them on-site to review the approved Scope of Work. This ensures the bids being placed are "apples-apples" comparisons. Bid reports are submitted to your case manager and a contractor is selected to complete your modification.
The contractors we collaborate with pride themselves on approaching every client with respect and dignity. While involved in your modification they carefully consider your circumstances, needs and available resources throughout the process.
Your Home Modification Assessor (HMA) will again meet with you and the contractor for a Pre-Construction meeting. This is where all the specific details of your modification will come together and allows the contractor to order the materials for your projects. The contractor will be able to give you timelines and estimated start dates during this meeting.
Our contractors make sure all materials are prepared and ready before your project begins, helping to keep the process efficient and on schedule. We know that construction can be disruptive, so we all strive to make it as smooth and stress-free as possible.
Your Home Modification Assessor (HMA) will manage your project from start to finish, maintaining clear communication with you, the contractor and your case manager throughout the process. If any change orders are needed, the HMA will handle the necessary submissions to keep the project moving forward. Once the work is complete, the HMA will conduct a final site visit to confirm that the Scope of Work was followed and ensure your satisfaction. Completion documents will be prepared for submission to the County, and you'll be able to enjoy the improvements made to your home.
